Dr Both has been providing care in northwest Ohio for the past 30 years.

Dr. Sudheer Polisetty is a graduate of Rangaraya Medical College in India. He has a Masters In Public Health from Missouri State University and an M.D in Internal Medicine from University of Nevada School of Medicine Las Vegas. He has worked as a board certified Internist/Hospitalist and has a decade of experience in acute inpatient care.

Dr. Polisetty was awarded as the Best Outgoing Resident for Ambulatory Care during residency, Patient Experience Champion and Most Involved Physician at UMC Hospital Las Vegas. He was also sub-investigator in many clinical research studies. He has a Fellowship in Geriatric Medicine from University of Toledo Medical Center.

Dr. Polisetty has a passion for supporting seniors in healthy aging and wellness, and provides a compassionate and dedicated service to all his patients.

Caitlin Roth is a board-certified nurse practitioner. She has worked as a provider for nine years in internal medicine in both skilled nursing and post-acute care.

She attended Case Western Reserve University for her bachelor’s degree in nursing and graduated in 2008. She continued her education at the University of Cincinnati and graduated with her master’s degree in 2013. She is board-certified by the American Nursing Credentialing Center (ANCC) as an adult-geriatric primary care nurse practitioner.
